These Terms & Conditions govern your request for a withdrawal or distribution from your

Individual Retirement Account (IRA). By executing this request, you acknowledge that you

understand the tax withholding instructions, and that Shape Plain Capital is not responsible for 

determining the appropriateness of your withholding, withdrawal, or distribution decision. You

understand that there may be tax consequences to this decision, including but not limited to tax

penalties.

You further understand that you must have sufficient cash in your IRA account for this request to

be processed. You expressly assume the responsibility of any consequence that may result

from the withdrawal or distribution, and you indemnify and hold harmless Shape Plain Capital and its 

affiliates, and their officers, directors, and employees.
